| INSURANCE AND PAYMENT
INFORMATION:
Call the office nearest you to
schedule an appointment. Allow about 1 hour for the first
consultation visit. This will allow
time for the appropriate allergy tests, if needed.
BEFORE THE CONSULTATION WITH THE
ALLERGIST:
Please come prepared with the
following information to make the visit, time-efficient and most
useful.
- List of all the medications
(including over the counter or non-prescription medications)
that you take.
- If possible, stop the allergy
medications that are taken in the form of pills or liquid
preparations to allow for possible testing. Ideally, oral
allergy medications should be held for 5 days or at least
for 3 days.
- You may continue nasal spray
and inhalers.
- DO NOT
STOP medications that are taken for other medical
conditions viz., heart medicines etc. If you have any
questions, please call the office.
- Bring a copy of any previous
laboratory test reports or any medical records.
HEALTH INSURANCE:
For health insurance plan payments,
please note the following:
- Bring your health insurance
plan card or other identifying information.
- Co-payment for the
visit will be accepted by check or cash.
- If you have a managed care
health plan, you will need a written referral (or
authorization) from your primary care physician
(Pediatrician, Internist or Family Practice doctor). It is
your responsibility to obtain the referral, before
consultation with the allergist.
Most of the health insurance plans
in Massachusetts are accepted. The following is a partial list of
the health plans.
- Blue Cross Blue Shield of
Massachusetts
- Cigna
- Fallon Health Plan
- Harvard Pilgrim Health Plan
- HCVM
- John Hancock health plan
- Masshealth
- Medicare
- Tufts Health Plan
- US Health Care
If you have any other questions,
please call the office. If you have no insurance coverage or you
are a direct pay (or self-pay), you can call the office for any
questions.
